All Participants must complete a proper entry form and submit this either by email register@giveatomarrow.org.uk or by post to arrive on or prior to the closing date.

If under16 years of age entrants must have an entry form approved by their parents or guardians.

If under 14years of age entrant must be accompied by an adult
Closing date for entries is 1st June 2008 or earlier if over-subscribed
ID Numbers

ID numbers will be dispatched to entrants prior to the event which will corresponde to your participant ID and MUST be worn by the entrant to whom the number has been registered.

Any participant whom has a medical condition, which may be affected by this event, must record it on the reverse of their ID Number.

ID numbers must NOT be exchanged with those of other entrants or used by anyone other than the original entrant.

No one without an ID number will be allowed to compete. |

I understand that if I am in any doubt about my physical or mental capability to take part in the Cambridge - Ampthill Cycle Challenge that I will seek the advice of my GP, consultant or other medical practitioner.
I understand that I am entering Cambridge - Ampthill Cycle Challenge at my own risk, taking full responsibility for my own fitness and safety, and that Give a ToMarrow, and its officers, employees or volunteers shall not be held liable for any death, injury or other loss occurring as a result of my taking part in Cambridge - Ampthill Cycle Challenge . I declare that I am medically fit to Cycle and understand that I enter at my own risk and that the organisers, their officers, employees or volunteers shall not be held responsible for any injury, loss or damage as a consequence of my participation in the event. If I have a medical condition which may be affected by my taking part in this event, I will record it on the reverse of my Member Competitor ID . I understand that this information is confidential and will only be used to ensure that I receive the correct treatment from the St Johns Ambulance should it be required. |
I understand that Give a ToMarrow may give out to organisations or individuals providing media coverage of Cambridge - Ampthill Cycle Challenge personal details and/or images of those taking part in Cambridge - Ampthill Cycle Challenge . I consent to my details and images being distributed and published in any media supplying coverage of Cambridge - Ampthill Cycle Challenge , including Anthony Nolan, Leukeamia research and the Willow Foundation own marketing and communications department.
I understand that this is a fundraising event and that in the event of a change of date for the event or cancellation of the event for any reason by the organisers that no refunds will be made in respect of the entry donation.
I understand that any funds raised will be divided equally between the three mentioned charities. |
